NURSE PRACTITIONER

Advanced Nurse Practitioner - Nexus Health Group, Southwark

Full Time - 37.5 hours/week

Salary – Band 7/8 depending on experience and qualifications

 

Nexus Health Group is an NHS practice in Southwark London; we provide primary care to over 70,000 patients from our 9 sites which are all based in North Southwark.

 

We are in the process of evaluating the way in which we provide care to our patients. Our business plan includes developing a service that will fit the needs of our patients that are in line with the NHS 5 year forward view and the aspirations set out in “better health for London”.

 

We are looking for an enthusiastic band (Advanced) Nurse Practitioner to work with us.  Our ideal candidate for this role will be experienced either in primary care and will be confident with first contact consultations and working at an advanced level of clinical practice.

 

The successful applicant will be qualified and experienced in working as an independent prescriber.

 

This is a full time post of 37.5 hours however, part-time hours will also be considered for the right candidate. We are offering a negotiable salary within the Band 7 / 8 range depending on qualifications and experience.

 

Overall Purpose of the role

 

 To deliver a high standard of patient care as Advanced Nurse Practitioner in general practice, using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base in line with the latest RCN guidance on Advanced Nurse Practitioner competencies.

 

Job Summary:

  • The Advanced Nurse Practitioner will work within their professional boundaries;
  • Provides care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
  • To undertake surgeries working in an autonomous role including the management of minor ailments, chronic disease, family planning, home visits to include both acute visits and the management of chronic medical problems in the house-bound
  •  To provide telephone based triage, assessments and consultations.
  • Demonstrates safe, clinical decision-making and expert care for patients within the general practice maintaining clinical excellence.
  • Works collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.
