Extensivist Advanced Nurse Practitioner– Care Home Residents - Quay Health Solutions, North Southwark
Salary range: Band 8A £51,383 - £57,596 - depending on experience
Full Time - 2 year fixed term contract
Quay Health Solutions GP federation are seeking to recruit an Advanced Nurse Practitioner with an interest in Frailty and co-morbidity for the delivery of an Extensivist role to work collaboratively within our Southwark GP Care Home Service to meet the needs of people living in nursing and residential homes.
This Extensivist role will play a key role in delivering Quay Health Solutions’ ambition to improve care for people living in nursing and residential homes in partnership with local care providers. Our service is focused on providing pre-emptive, coordinated care so that care is provided more proactively to improve or maintain people’s health and wellbeing.
This is an exciting new role which will offer the successful candidate the opportunity to be innovative, make informed decisions and develop this new role within the multidisciplinary care home team, while also involving care home staff and public to ensure agreed standards, targets and objectives are met.
You will be a nurse who will be able to demonstrate your clinical expertise and experience in managing patients with multiple long term conditions and act as a clinical leader within the Southwark GP Care Home Service. You will autonomously assess care home residents using an agreed comprehensive geriatric assessment (CGA) and demonstrate safe, senior clinical decision-making and expert care for residents providing appropriate clinical management plans and prescription of any urgent medication required.
You will be able to demonstrate positive clinical leadership within a multi-disciplinary team, being accessible and approachable, working closely with colleagues in the Southwark GP Care Home Service and the wider care teams in the care homes, community and social services, local acute and mental health trusts, along with any relevant voluntary care organisations that support this group of people.
