Community Care Nurse

Nexus Health Group is an NHS practice in Southwark London; we provide primary care to over 70,000 patients from our 9 sites which are all based in North Southwark.

We are in the process of evaluating the way in which we provide care to our patients. Our business plan includes developing a service that will fit the needs of our patients that are in line with the NHS 5 year forward view and the aspirations set out in “better health for London”.

We are looking for a Community Care Nurse to work with us in developing our ambitious plans of providing primary health care to a large population. We are looking for an individual with enthusiasm for change and service development as we build the key components of an integrated system of care for elderly, frail and patients with long term conditions. The successful candidate will be the lead in this area in Decima Street and The Artesian Health Centre.

This is a full time post of 37.5 hours we are offering a salary between £30,000 to £36,644 depending on qualifications and experience, plus NHS Pension.

Overall purpose of the role

  • To assess patients with complex long term conditions, severe and moderate frailty and elderly; in the patients home and surgery setting.
  • Co-ordinate and plan the care of those patients identified and refer them to the appropriate MDT and outside agencies and stakeholders to support this work; so that the patient receives timely intervention.
  • To innovate with the vision, motivation and compassion to drive the service forward as the recognised clinical lead for this locality service.
  • To provide gold standard care for our patients in the latter part of their lives.
  • To use specialist knowledge in assessing, planning and implementing packages of care.
  • To demonstrate high level problem solving and decision making skills.

Qualification

  • NMC Registered Nurse (Adult)
  • Current NMC Qualification
  • Post registration courses in chronic disease management

Experience

  • Minimum of 2 years post graduate experience in a community or primary care setting
  • Working within a multi-disciplinary team
  • Experience of managing a case load, implementing care pathways, protocols and clinical guidelines.
  • Experience of management of complex long-term conditions and can demonstrate the importance of chronic disease management and rehabilitation in the community

 We are looking for a highly motivated and enthusiastic Practice Nurse to work with us and successful candidate must be able to perform all the standard Practice Nurse duties.

 This is a full time post of 37.5 hours we are offering a salary between £32,000 to £35,000 depending on qualifications and experience, including NHS Pension.

 

Overall Purpose of the Role:

The general practice nurse is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. The focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. The post holder will work collaboratively with the whole general practice team and report to the locality lead nurse. 

Experience

  • Previous primary care experience and knowledge of the working of general practice
  • Clinical skills – cervical cytology, immunisation and vaccination, ear care, minor surgery
  • Experience of managing chronic disease clinics
  • Change-management skills and ability to support patients to change lifestyle
